Berberine Hydrochloride: 750 mg, Myo Inositol: 550 mg, D-Chiro Inositol: 13.8 mg,  L-Methyl folate: 300 mcg, Chromium as Chromium  Picolinate: 24.86 mcg, Vitamin D2 (Ergocalciferol) 400IU.
