Jitorab LS Capsule Overview:

Jitorab LS Capsule is a combination medicine mainly used to treat long-term digestive disorders such as Gastroesophageal Reflux Disease (GERD). It is recommended when a single medicine, especially a proton pump inhibitor (PPI), does not provide sufficient relief or when patients do not respond adequately to PPI therapy alone.

GERD is a condition in which stomach acid and partially digested food flow back into the esophagus (food pipe). This backward flow, known as acid reflux, often causes a burning sensation in the chest, commonly referred to as heartburn.

Jitorab LS 20/75 Capsule contains Rabeprazole and Levosulpiride. Rabeprazole works by reducing the production of stomach acid, while Levosulpiride helps improve the movement of the digestive tract (gut motility). Together, they help relieve symptoms by decreasing acid levels and ensuring smoother passage of food through the digestive system.

Composition:

Each hard gelatin capsule contains- Rabeprazole Sodium (as enteric coated pellets): 20 mg and Levosulpride (as sustained release pellets): 75 mg.

Colours: Red Oxide of Iron & Titanium Dioxide

Approved colours used in capsule shells.

Uses & Benefits of Jitorab LS Capsule:

Jitorab LS 20/75 Capsule is primarily used to manage the following condition:

  • Gastroesophageal Reflux Disease (GERD): It effectively relieves acid reflux and "heartburn," the painful burning sensation caused when stomach acid flows backward into the esophagus (food pipe).

How Does Jitorab LS Capsule Work:

Jitorab LS 20/75 Capsule works through a dual-action mechanism with its two active ingredients.

Rabeprazole is a selective Proton Pump Inhibitor (PPI) that works by blocking the proton pump in the stomach lining, which is responsible for producing gastric acid. Inhibiting this enzyme, it effectively reduces stomach acid production, lowering acidity and helping to treat conditions like GERD, acid reflux.

Levosulpiride acts on dopamine receptors and enhances the movement of the stomach and intestines, which helps in reducing symptoms like bloating, discomfort, and delayed gastric emptying. Together, they provide effective control over acid reflux and digestive disturbances.

Dosage of Jitorab LS Capsule:

  • Usage Direction

Take Jitorab LS Capsule strictly according to your doctor’s instructions, following the prescribed dose and timing. Jitorab LS 20/75 should be swallowed whole and not opened, chewed, or crushed, preferably at the same time each day. Do not stop or alter the dose of Jitorab LS Capsule without consulting your doctor, as this may prevent you from achieving the desired outcome. If you are unsure about the administration procedures of Jitorab LS, consult with your doctor or pharmacist.

  • Missed Dose

If you miss a dose of Jitorab LS 20/75 Capsule, take it as soon as you remember on the same day. But, if it is almost time for your next scheduled dose, skip the one you missed and continue following your regular dosing schedule. Do not attempt to take two doses if you have forgotten one. If you often forget to take your medication, consult with your doctor to re-evaluate your dosing schedule. Frequently missing doses could lead to undesired outcomes.

You can set reminders on your smartphone, keep drug organizer boxes, or use other audio-visual technologies to prevent missing a dose.

  • Overdose

If you accidentally take more than the prescribed dose of Jitorab LS 20/75 Capsule, you must seek immediate medical attention. Symptoms of overdose may include dizziness, drowsiness, confusion, or gastrointestinal discomfort.

Contraindications of Jitorab LS Capsule:

Do not take Jitorab LS 20/75 Capsule if you are hypersensitive or allergic to Rabeprazole, Levosulpiride, other proton pump inhibitors (such as omeprazole or pantoprazole), or any other ingredients of the medicine.

This medicine must not be taken by patients with epilepsy, a history of seizures, or Parkinson’s disease. It is also unsafe for those dealing with acute digestive emergencies, such as gastrointestinal bleeding, mechanical obstructions, or perforations (tears in the stomach or intestinal wall).

Additionally, patients with severe kidney or liver impairment, certain adrenal gland tumors (Pheochromocytoma), or mental health conditions like bipolar disorder and severe depression should avoid this treatment.

Precautions & Warnings of Jitorab LS Capsule:

The use of Jitorab LS 20/75 Capsule requires careful medical supervision, particularly during extended therapy. Prolonged use can interfere with Vitamin B12 absorption, cause atrophic gastritis, and may lead to low magnesium levels (hypomagnesemia).

Inform your doctor if you have liver or kidney disease, as the use of Jitorab LS requires caution in these cases. Long-term use (over a year) may increase the risk of bone fractures (especially in the hip or spine); thus, patients at risk for osteoporosis should be monitored closely.

Due to the dopamine-antagonistic properties of Levosulpiride, it may cause movement disorders (extrapyramidal symptoms) or mood changes, so use with caution.

Even if you feel better after taking this medicine, it does not mean serious problems like gastric malignancy are not present, so if symptoms continue or get worse, consult your doctor.

Side Effects of Jitorab LS Capsule:

Jitorab LS Capsule is generally well tolerated, but some people may experience mild side effects such as headache, dizziness, dry mouth, stomach pain, gas, or diarrhea. These effects are usually temporary and tend to improve as the body adjusts to the medicine.

In rare cases, some serious side effects may also occur. These may include hormonal changes like breast tenderness, mood changes, or a sudden drop in blood pressure when standing (called postural hypotension), which can cause dizziness. Some people may also experience muscle stiffness or skin rash.

If you notice signs of an allergic reaction or Jaundice (such as yellowing of the eyes or skin), or if any side effects become severe or do not go away, seek medical help immediately.

Safety Related Concerns of Jitorab LS Capsule:

  • Pregnancy

Jitorab LS Capsule is not recommended during pregnancy unless clearly needed and prescribed by your doctor. If you are pregnant or planning to become pregnant, talk to your doctor before taking this medication to weigh the benefits against potential risks to the developing baby.

  • Breastfeeding

It is better to avoid the use of the Jitorab LS 20/75 Capsule while breastfeeding, since the active ingredients of this medicine could pass into the breast milk and affect the breastfed child. Therefore, consulting with your doctor before taking Jitorab LS is crucial.

  • Alcohol

It is strongly advised to avoid consuming alcohol during treatment with Jitorab LS Capsule. Levosulpiride can affect the brain, and alcohol may increase side effects like drowsiness, poor coordination, and reduced alertness.

Alcohol can also increase stomach acid, which may reduce the effect of Rabeprazole and worsen symptoms like heartburn or ulcers.

  • Food Interactions

Jitorab LS Capsule is typically advised to be taken on an empty stomach to maximize its effectiveness. The interaction between specific food items and the Jitorab LS 20/75 Capsule is not listed. However, certain foods can trigger stomach acid, so avoid spicy, fatty, oily, or highly acidic foods (citrus, tomatoes), and caffeine or carbonated beverages.

Following these dietary recommendations helps to avoid worsening acid-peptic symptoms and aids in the mucosal healing process that the medication starts.

  • Drug Interactions

Jitorab LS Capsule may interact with a range of other medications, potentially altering their effectiveness or increasing the risk of adverse effects. Rabeprazole can reduce the absorption of medicines that depend on gastric acidity for uptake, including ketoconazole, itraconazole, atazanavir, and iron salts. It significantly reduces plasma levels of rilpivirine (an antiretroviral), making the combination contraindicated. Rabeprazole may also interact with warfarin, methotrexate, and digoxin, requiring close monitoring.

Levosulpiride may interact with antipsychotic medicines, other dopamine antagonists, and medications known to prolong the QT interval (such as certain antibiotics, antiarrhythmics, and antifungals), raising the risk of serious cardiac side effects. Concomitant use with anticholinergic drugs may counteract Levosulpiride's prokinetic effects.

Always inform your healthcare provider and pharmacist about all medications you are taking, including prescription drugs, over-the-counter medications, vitamins, minerals, and herbal supplements, to avoid potential interactions.

  • Using Machinery & Driving

Jitorab LS Capsule may cause side effects like drowsiness, dizziness, numbness, or visual disturbances in some people. Patients should use caution and avoid driving or operating machinery until they are sure the medication does not affect their alertness.

Storage of Jitorab LS Capsule:

Store protected from light & moisture, at a temperature not exceeding 30°C. Keep medicines out of reach of children.

Avoid using it after the expiry date, printed on the packaging. Dispose of expired or unused medication properly; do not throw it away casually.

Additional Information:

  • Eat smaller, more frequent meals rather than three large ones. Do not lie down immediately for at least 2–3 hours after eating.
  • If experiencing nighttime acid reflux, use a wedge pillow to keep your head elevated.
  • Smoking increases acid secretion; quitting can significantly improve the medicine's success.
  • Eat fiber-rich foods and stay hydrated to support gut motility.
  • Do not suddenly stop your medication unless your doctor specifically tells you to.
  • Do not share your medicine with someone else having the same symptoms as you.

Frequently Asked Questions (FAQs):

Q. What is Jitorab LS 20/75 Capsule primarily used for?

A. Jitorab LS 20/75 Capsule is primarily used to treat Gastroesophageal Reflux Disease (GERD) and its associated symptoms, particularly when standard acid-blockers alone aren't enough.

Q. Can I take Jitorab LS Capsule after meals?

A. Take Jitorab LS Capsule exactly as your doctor instructed, without skipping any doses, preferably at the same time each day. However, it is generally recommended to be taken on an empty stomach, at least 30 minutes before, for maximum absorption.

Q. How long can I take Jitorab LS Capsule?

A. Jitorab LS 20/75 Capsule should be taken strictly for the duration prescribed by your healthcare provider, as it is primarily intended for short-term symptomatic relief. Extended use (typically exceeding one year) could lead to certain risks, including a higher incidence of osteoporotic bone fractures (hip, wrist, or spine), low levels of magnesium and vitamin B12. Patients on long-term therapy require periodic medical assessment and laboratory monitoring of mineral and vitamin levels to ensure continued safety.

Q. Does Jitorab LS Capsule lead to weight gain?

A. Yes, Jitorab LS Capsule can cause minor changes in body weight for some patients. This is primarily due to the active ingredient Levosulpiride, which can occasionally affect metabolic processes and hormonal balance (specifically prolactin levels), leading to slight weight gain. While minor changes are usually not a cause for concern, you should monitor your weight during treatment. If you experience significant or rapid weight gain, please consult your healthcare provider immediately to determine if a dosage adjustment or an alternative therapy is necessary.

Q. Does Jitorab LS 20/75 Capsule cause sleepiness?

A. Yes, Jitorab LS Capsule can cause sleepiness or drowsiness in some patients. This is primarily due to the active ingredient Levosulpiride, which affects certain chemical messengers in the brain and can have a sedative effect. Because of this, you should exercise extreme caution when driving, operating heavy machinery, or performing tasks that require full mental alertness until you know how the medication affects you. To minimize safety risks, it is also advised to avoid alcohol, as it can significantly worsen this drowsiness.

Q. Can children take Jitorab LS Capsule?

A. Jitorab LS 20/75 Capsule is generally intended for adults and is not recommended for children. While it may be considered for adolescents over the age of 14 who are suffering from severe GERD symptoms, this must only be done under strict pediatric guidance. The safety and efficacy of the active ingredients, particularly Levosulpiride, have not been extensively established for younger pediatric populations. You should always consult a child specialist or pediatrician before administering this medication to anyone under the age of 18 to ensure it is the safest course of treatment.
